Last date for Cash Payment at Bank for Online application March 08, 2014
Last date for receiving completed application online up to 5:00 PM March 10, 2014

Expected date of declaration of merit list: April 04, 2014
Date of Medical Board for obtaining physically handicapped certificate: April 10, 2014
Tentative Date of commencement of first counselling: April 22, 2014 (Venue: K.G.Medical University. Lucknow).
Last date for joining the allotted seat from first counselling: April 29, 2014
Date of Start of Course: May 01, 2014
Tentative Date of commencement of second counselling: May 13, 2014
Last date for joining from 2nd round of counselling: May 20, 2014

Important Information:
1. Please go through the brochure carefully before filling the form. You may download the brochure from the website.
2. Application can only be made online only. There is no paper application.
3. Candidates paying fee through cash at Axis Bank/Allahabad Bank can fill online application only after 2 days of making payment. Last date for making cash payment is Saturday, 08th Mar 2014
4. Candidates making payment through Internet payment gateway (debit/credit card) mode can apply instantly up to 5:00 PM. on 10th March 2014.
5. A non-refundable application fee of Rs 1800/- + Bank Transaction Charges shall be paid by the candidate. Bank charges shall be –
i) Cash deposit: Axis Bank: Rs 50/-; Allahabad Bank: Rs 20/-
ii) Internet payment: Debit Card 0.75% + Service Tax; Credit Card 1.5% + Service Tax
6. Candidates are required to assess their eligibility, as given in the eligibility section of this information brochure, themselves before applying. The examining body will not assess their eligibility. The eligibility will be checked from original documents at the time of counselling/admission and candidates found not eligible will be denied admission for which they themselves will be responsible.
7. Last date for receiving application online is Monday, 10th March 2014 up to 5:00 PM.

Important Note regarding photograph :
(Comply to these very carefully as any deviation will lead to rejection of form.)
Photographs should be
1. High contrast 3.5 × 4.5 cm size colour photographs (full front face, no cap, no hat, no dark glasses) with white/light background. Top of hair to bottom of chin should measure 2.5 to 3.0 cm.
2. Poor quality photographs would be rejected.
3. Photograph MUST be taken on or after 01.01.2014.
4. Photograph must have clear and legible name and date of the candidate on the photograph.
5. Instruct your photographer to take the photograph with a 4MP or better camera and provide you a soft copy of the photograph in “.jpg” format. The file size should be between 250Kb to 500 KB.
6. A sample photograph is given on website.
