Service Description :
Applicant can fill an application e-form application for change of name in death certificate.

How to apply :
The applicant who wants to apply for change of name in death certificate shall submit the e-form duly filled-in. He/she shall state true facts in the application. Follow below instructions to access the application,
1. Click on Services and Forms on the portal.
2. From the department list, select Local Administration.
3. List of all services will be displayed. Depending on the method of submission, click one of the links under Action.
4. Fill the online form, attach supporting documents and submit.
5. Acknowledgment with unique registration number will be displayed.
6. Using the unique registration number, the user can check the status of the application on the state portal.
7. Once the status is complete the user has to meet the concerned department person along with the required original documents and collect the certificate from the department person.

The following supporting documents are required to be submitted along with the e-form.
Maximum file size should not be more than 2MB.

1. Identification Proof of the Deceased.
2. Residence Proof of the Deceased.
3. Affidavit.

5. Any other documents.
