National Disability Certificate and Identity Card :
To facilitate rehabilitation services for the mentally challenged persons, children with autism, cerebral palsy and multiple disabilities, the Persons With Disability Act (PDA) 1995, and National Trust Act, 1999 provides guidelines and instructions to the Central and State Government to provide rehabilitation schemes. On the ground of these acts, the Chandigarh Administration provides National Disability Certificate and National Disability Identity Card to the persons with mental retardation, cerebral palsy, autism and multiple disabilities through Govt. Institute for Mentally Retarded Children (GIMRC), Sector 32, Chandigarh, India.
Aim :
In December 2001, GIMRC, Chandigarh, took the challeng to save the disabled persons and their families from the hassles and running around for the disability certificate. The GIMRC, provides all the required formalities related to obtaining the certificate under one roof.
Procedure :
A board sits on every Wednesday to assess the application forms for certificate and Identity Card.The board consists of Special Educator, Clinical Psychologist, Officer-in-Charge of the institute and Joint Director of the institute. The certificate is counter signed by the Director Health Services, Chandigarh.
The documents required for the disability certificate are :
** Passport size photographs
** Date of Birth Certificate
** Blood Group Report
** I.Q. Certificate issued by Clinical Psychologist.
On the basis of National Disability Certificate the yellow coloured, National I-Cards are issued to the persons from Chandigarh by Director Social Welfare, Chandigarh. To acquire this I-Card, applicant has to produce resident proof of Chandigarh.

** Concession in Income Tax
** The Social Welfare benefits or facilities provided by State Government through Sate Social ** Welfare Departments
** The parents of major disabled children can acquire guardianship certificate.
Duration :
The I-Card is valid for 5 years for minor candidate and after 18 years of age, permanent I Cards are made. National Disability Certificate is made once for all. The Certificate and I-Cards are provided free of cost.
Approximate time for getting the certificate is maximum one month after the submission of complete application form.
