** Lost card needs to be reported at 040 – 44885000. Krishna Institute of Medical Sciences Ltd., doesn’t hold any
** responsibility for any unauthorised usage of the lost card.
** The amount is accepted ONLY through CHEQUE / DD / RTGS.
** The billing will be done as per the hospital tariff applicable to each location at that given point of time.
** No cancellation is entertained once the card is swiped and the amount is deducted.
** The hospital tariffs will be revised in the month of April every year.
