Digital Statutory Forms(C, F, H, E1 and E2):
1. Application of Statutory-form :
Registered user need to login to MPCTD web portal and apply for the Statutory-Form as per the process explained below
1. Click on ‘Statutory Form Request’ from the left menu
2. Enter the required detail( You can select the type of form C, F, H E-1 and E-2 from the Form Type dropdown. Process is same for all the forms )
3. After submission of request, following message would be displayed. Please save the request id once it is generated
4. For uploading purchase details click on ‘’Statutory Forms-Purchase Upload’’ as shown below and download the template for it from portal
5. Click on ‘Click here’ to download the template
6. Save the template on your computer (Note- Enable the macros before entering the details in the template)
7. Enter the required details in the correct format in the excel sheet
8. Click on ‘Validate’ to generate the upload file. Once generated save it on your computer
9. Now enter the request id and upload the purchase details as shown below and submit it
10. Now Click on Submit button
11. Following message would be displayed after submitting of purchase details. (Note- Statutory-Form related Approval/Rejection mail would be received in your ‘Received Message’ link. Once form is approved by the Authorized CTD officer , you will receive a mail regarding approval in your portal mail box as shown below)
12. Click on ‘Received Message’ link from the left menu. Approval mail screen would be shown as below

2. Download Digital Statutory Form :
1. Once Form is approved by the respective officer, you need to click on the ‘Approved Statutory Form’ link on the left menu and enter the parameter Form Type, Quarter and Year. Click Search
2. Once click on Search , following screen would be displayed Click on ‘Generate’ button. Statutory form in pdf format will be opened up. You can save it on your computer. Once statutory form is generated, you can reopen/download the statutory form from portal anytime. You need to validate the digital signature as per the steps mentioned in the next chapter

3. Validate Digital Signature :
1. Open Statutory-form pdf in acrobat reader. It will show the “Validity Unknown” in Digital Signature as show in below screenshot (Note- If you are e-mailing digital signature verified form pdf to some other dealer in such cases, other dealer will also need to verify the digital signature once on his computer as per the steps explained below. Next time onwards digital pdf form will be automatically signed on his computer)
3. Right Click on Digital Signature and Click on Validate Signature. Click on Signature Properties.
4. Click on Signer and then click Show Certificate
5. Click on Trust.
6. Click on the Add to Trusted Identities.
7. Tick all checklist and Click on OK.
8. Click on Validate Signature.

Now you can see Certificate with Valid Signature. This exercise is required to perform Only once on a computer. Next time, all Digital statutory-Form which are downloaded on that computer will be automatically signed on that computer
