Janata Personal Accident Insurance Policy :
** JPA policy covers any person irrespective of sex, occupation and profession in the age group of 10 to 70 years for death and permanent total disablement and permanent partial disablement.
** The policy is available for minimum sum insured of Rs.25.000/- per person per annum and the maximum sum insured is to be limited to Rs.1,00,000/- per person per annum.
** The rate of premium is Rs.15 for a sum insured of Rs.25,000/-. The sum insured shall be increased in multiples of Rs.25,000 and premium is charged accordingly.
** The policy is also available for pre-identified groups as listed by the scheme. In case of a group policy group discounts are available as per size of the group.
** The policy is also available for a long term period up to 5 years

Key Benefits:
This insurance will pay to the Insured (or in case of death, to the nominee) the amount shown against the table of benefits, if the Insured shall sustain any bodily injury resulting solely and directly from accident caused by outward violent and visible means and such bodily injury within the twelve calendar months of its occurrence be the sole and direct cause of:

The various coverage are explained as below:
1. Accidental Death:

It provides for payment of compensation on the insured person’s death caused by bodily injury arising out of accidental, violent, external and visible means and resulting in death within 12 (twelve) calendar months of occurrence of the accidental injury.

2. Permanent Total Disablement:
It provides for payment of compensation for bodily injury caused by accidental, violent, external and visible means resulting in Permanent Total Disablement resulting from an accident.

3. Permanent Partial Disablement:
It provides for payment of compensation for bodily injury caused by accidental, violent, external and visible means resulting in Permanent Partial Disablement within 12 (twelve) calendar months of occurrence of such injury.

Conditions :
1. Upon the happening of any event which may give rise to a claim under the Policy the Insured shall forthwith give notice hereof to the Issuing Office of the Company unless reasonable cause is shown the Insured or his legal heirs should within one calendar month after the event which may give rise to a claim under the policy give written notice to the Company with full particulars of the claim.
2. Proof satisfactory to the Company shall be furnished of all matters upon which a claim is based. Any medical or other Agent of the Company shall be allowed to examine the person of the Insured on the occasion of any alleged injury or disablement when and so often as the same may reasonably be required on behalf of the Company and in the event of death to make a post mortem examination of the body of the Insured and such evidence as the Company may from time to time require (including a post mortem examination if necessary) shall be furnished within the space of fourteen days after demand in writing and in the event of a claim in respect of loss of sight the Insured shall undergo at the Company’s expense such operation or treatment as the Company may reasonably deem desirable provided that in the case of valid claim arising under sub-clause (a) (b) or (d) all sums payable hereunder shall be payable only on the delivery of this Policy cancelled and in case of valid claim arising under sub-clause (c), the policy should be submitted to the insurer for recording payment of claim.
3. Claims of person presume to be dead due to drowning, only be settled after two years of the submission of the following documents.
4. Under no circumstance claims under such policies can be paid to any one other than the individual insured or his beneficiary/beneficiaries.
5. No sum payable under this Policy shall carry interest.
6. The Company shall not be liable to make any payment under this policy in respect of any claim if such claim be in any manner fraudulent or supported by any fraudulent statement or device whether by the Insured or by any person on behalf of the Insured.
7. The Company may at any time by notice in writing cancel this Policy provided that the Company shall in the case return to the Insured the then last paid premium less a pro-rata part thereof for the portion of current insurance which shall have expired. Such notice shall be deemed sufficiently given if posted addressed to the Insured at the address last registered in the Company’s book and shall be deemed to have been received by the Insured at the time when the same would be delivered in the ordinary course of post.
8. No refund of premium will be allowed even if the claim arises in the earlier years under long term policy
9. If the Insured shall at any time during the Policy be insured against similar Janata Personal Accident Insurance Policy with one or more Insurers then the maximum liability of the Insurers irrespective of the number of such Policies in force with one or more Insurer shall be limited to maximum admissible Sum Insured to be determined by the insurer from time to time for an individual depending on his annual income.
10. If any difference shall arise as to the quantum to be paid under this policy (liability being otherwise admitted) such difference shall independently of all other questions be referred to the decision of an arbitrator to be appointed in writing by the parties in difference or if they cannot agree upon a single arbitrator the decision of two disinterested persons as arbitrators of whom one shall be appointed in writing by each of the parties within two calendar months after having been required so to do in writing by the other party in accordance with the provision of the Arbitration Act, 1940 as amended from time to time and for the time being in force. In case either party shall refuse or fail to appoint arbitrator within two calendar months after receipt of notice in writing requiring an appointment the other party shall be at liberty to appoint sole arbitrator and in case of disagreement between the arbitrators the difference shall be referred to the decision of an umpire who shall have been appointed by them in writing before entering on the reference and who shall sit with the arbitrator and preside at their meetings.

** Collect relevant Pre printed Proposal Form.
** Read and Fill in the Form correctly.
** Submit the Proposal Form to our Office either directly or through an authorised Agent. Please note that our Agent service can be availed by directly contacting our office concerned through either telephone lines or through E-mail.
** Some risks may call for pre-acceptance survey. In such cases surveyor will be appointed by our office to estimate the quantum of risk involved and other related matters.
** Premium estimates will be given to the Proposer through our Computerised Offices.
** Policy will be issued to our customer either directly on request or through registered post
