Birth /Death Certificate :
The Health Department is an administrative department of MCGM which is responsible for Birth/ Death registrations and issue of Birth/Death Certificates. It also issues license in consultation with an advice from other technical advisory departments like, Fire Brigade, Health, Engineering, Estate, etc. In view of this it is necessary for Establishment to take necessary license from Heath department before starting manufacturing, storage or trade of any commodity/activity. The application must be received from the establishment three months prior to starting the activity. Commodities/ activities including eating house, restaurants, manufacturing drugs, medical shops, manufacturing and/or sale of milk and milk products, condiments, aerated water, ice factories, etc. fall under this license category.

Where to Apply? :
Visit the nearest Citizen Facilitation Center (CFC) established by the Municipal Corporation belonging to any of the 24 wards of MCGM. Alternatively, for few applications such as application for Birth/Death Certificate, fill in online application forms from any of the computer connected to Internet, either from home/office or cyber café by visiting our Citizen Portal provided applicant’s birth is already registered with MCGM.

I am unable to find my birth records. What should I do? :
You may contact your nearest ward for further help in this matter. Please note, the birth records after the year 1996 are available in the System but the records prior to 1996 may not be available in the system as of now.

How can I check the records and my information? :
Please input the Registration Number, a number provided to you by the Hospital/ Nursing Home/ Maternity home; for searching your Registration record in the system.

I am not sure of my registration number. Can I provide other details? :
In case you are unable to put the registration no. you may provide the other search criteria such as Gender, Date of Birth, Ward, Father’s Name, Mother’s Name or Hospital Name

I want to name my child. What is the procedure for the same? :
In case you want to name your child, first, enter the additional data to authenticate your identity. You may then input any one of the fields i.e. Pan Card no., Driving License No., Voter’s card No., Passport No., Ration Card No. or Mother’s maiden Name. Please ensure that the data provided by you matches the information provided at the time of registration

Note :
A child’s name once entered and saved cannot be changed. To avoid this please confirm the name you would like to enter in the form before submitting the form

How do I specify the number of copies I need? :
You may enter the no. of copies you wish to have in the “No. of copies” section as indicated.

Where do I have to collect it from? :
Please enter the mode of delivery i.e. whether you would like to collect the same from the ward or have it couriered to your address. If you have opted to take the delivery of certificate through courier, the same will reach you within the days stipulated by the Department of Posts. Please re?confirm the Delivery address before submitting the form in case of mode of delivery ? courier.

Do I have to pay a fee for this application? : If yes, how can I make the payment? :
You can make online payment by using Net banking facility from the listed banks through the Payment gateway. Once the payment is made online, please make a note of transaction id that is generated

I want to review the form before submission and check if all details are entered correctly. How can I do so? :
Before submitting the form, a preview of the form can be done using the “Preview” Button provided in the form.

I want to make changes to my form. What should I do? :
Click on Preview to view the form. On the preview page, go to “Edit” and click to make your changes.

I want to print the form. How can I do so? :
Once you have submitted the form, the printable version will appear. You can now print it.

How to Apply at the CFC? :
Applicant needs to submit the duly signed application form along with required documents at the CFC. The CFC agent fills in the application in the computer system (provided applicant’s birth is already registered with MCGM). The CFC agent also collects necessary fees (as per number of copies) and required documents from the applicant. As a final step the CFC agent prints and issues Certificates (as per number of copies) and fee receipt to the applicant.

  • I was born in Nair hospital. At the time of after black out my father told me that black out was in the year 67. I don't have my birth certificate but it is recorded.
    So how will I get my birth certificate?

  • I have been visited Ward C in Charni Road behind the Garden BMC office 3rd floor. I had also applied for my Birth Certificate giving my parent's residential address at that time. (The reason for applying for a copy of my Birth Certificate is that my original one was submitted at my college and they could not retrieve the same and am not able to travel to Mysore).
    My details DOB:07.10.1953 at Dhobi talao. Thankful for your help.

  • I need to get the death certificate of my grand father. I already had one original certificate and that was lost now. The location comes under Mumbai municipal corporation. Can I apply for the new one if i provide the details? Also I don't find any online application right now.Please suggest.
